Bone augmentation

Bone augmentation Wuppertal. Lack of bone volume in the area of the dental defect is one of the most common relative contraindications to implantation. Statistically, 70% of dental implants are placed in conditions of jawbone atrophy of varying severity. To solve this problem, many patients are prescribed some kind of bone grafting.

Why bone grafting is necessary

The roots of natural teeth are located in the cancellous part of the jawbone, called the alveolar process. This area of the jaw is riddled with blood vessels and nerves. The parameters of the alveolar processes are not constant values and change under the influence of various factors – the intensity of masticatory load, traumatic tooth extraction, endocrine or systemic diseases of the joints and bones. Consider how the absence of chewing pressure on the jawbone due to tooth extraction affects the jawbone.

The roots, thanks to the constant chewing load, keep the alveolar processes toned. When one or more teeth are lost, the jawbone in the area of the dental defect is no longer under pressure and gradually atrophies. Bone loss, or resorption, begins several months after tooth extraction and can continue up to the complete loss of the original volume.

Depending on the localization of the defect, bone resorption occurs differently. Because the density of the tubular bone in the upper jaw is several times lower than in the lower jaw, the atrophy of the upper alveolar process occurs much faster. So, if the lower jaw bone shrinkage process starts after 6-12 months, the upper jaw – already 2-3 months after tooth extraction. Bone augmentation Wuppertal

Dentists distinguish the following types of bone atrophy:

  • The alveolar process is resorbed in width (horizontal atrophy).
  • The alveolar process is resorbed in height (vertical atrophy).
  • Resorption occurs in width and height simultaneously (combined atrophy).

Depending on the direction, degree and localization of resorption, different techniques of bone augmentation are used for dental implants. The chosen technique of bone grafting in turn determines the type of osteoplastic material for bone augmentation.

Types of Bone augmentation grafting for dental implants

1. Splitting of the alveolar process

Description of the technology: the alveolar process in the area of the dental defect is sawed along the ridge with a dental drill. The hole is enlarged with drills of different diameters, and then root implants are screwed in. The cavities remaining on both sides of the rods are filled with synthetic or natural osteoplastic filler, covered with a resorbable membrane, and the gingival flap is sutured.

Three to six months after tissue regeneration is complete, abutments and temporary dental crowns are placed on the implants. Bone augmentation Wuppertal

Indications: This bone grafting technique is used to restore several teeth with horizontal resorption of the alveolar process with a thickness of at least 2 mm.

Important: the method allows for bone grafting simultaneously with dental implants, promotes rapid regeneration of bone tissue, does not require surgery for the intake of autologous bone material.

2. Bone augmentation block transplantation

Description of technology: the operation is carried out in two stages.

  1. At the first stage, donor bone is harvested from the chin area, from the cusps of the missing wisdom teeth on the upper jaw, and in rare cases from the iliac bone.
  2. At the second stage the autogenous bone material is implanted into the desired area of the jaw on titanium screws. After that, the regenerated area is closed with a two-layer resorbable membrane and concealed under the gingival flap.

Indications: restoration of one or more teeth with severe bone resorption in width or height.

Important: Using your own bone greatly increases the chances of successful regeneration of the grafted unit. This type of bone grafting does not allow you to immediately proceed to implantation. The implant is not placed until 6-12 months after the first surgery.

3. Guided bone regeneration

Description of the technology: the gingiva is opened, after which the area is introduced a natural osteosubstituting material based on bovine bone. The area is covered with a two-layer collagen membrane for directed bone regeneration and sutured. An unresorbable film on a titanium mesh frame can be used instead of a resorbable membrane.

Indications: Used for mild to moderate atrophy of bone tissue in width and height.

Important: augmentation can be performed simultaneously with the placement of the implant and does not require a bone grafting operation.

4. Sinus lift

Description of the technology: During the operation, the surgeon-dentist raises the bottom of the maxillary sinus, and the resulting cavity is filled with a synthetic bone filler. Depending on how the manipulation to correct the floor of the maxillary sinus is performed, there is a distinction between closed and open sinus elevators. Bone augmentation Wuppertal

Indications: Upper jaw in the area of 5-7 chewing teeth with close location of maxillary sinuses.

Important: In most cases, implants on the upper jaw are performed simultaneously with a sinus elevator.

How to avoid bone grafting

Tsypin Dental Center offers clients several alternatives to bone grafting. To avoid complications and if indicated, we can perform dental implants without bone augmentation in the following ways:

  • to place compression-type implants into deeper layers of bone;
  • Place long root canal implants at an angle to the tooth plane or bypass the maxillary sinuses;
  • perform a full jaw implantation on four or six implants.

About lower jaw Bone augmentation grafting

Implants are the most advanced and aesthetic type of prosthetics and are successfully used to restore the integrity of the tooth row in the absence of one or more teeth.

Often there is a lack of bone in the jaw as a result of various injuries to the jaw, complications from failed tooth extractions, diseases (granuloma, periodontitis) or atrophy from a long absence of a tooth. In such cases, dental augmentation is required.

Bone augmentation in dentistry helps to restore the shape and aesthetic position of the jawbone, making it possible to carry out successful dental prosthetics by placing an implant. Bone augmentation Wuppertal
