What is tooth decay?

Decay develops when bacteria in the plaque turn sugar into acid. That acid gradually dissolves minerals out of the enamel until a gap forms — the hole in the tooth. In the beginning this often happens without any pain at all. The earlier we spot it, the more gently it can usually be treated and the more healthy tooth substance is preserved.

Decay usually announces itself quietly. These are the signs worth having checked, precisely because early decay often does not hurt yet.
White or brownish spots
Chalky white or discoloured patches on a tooth can be an early sign that decay is starting. At this stage a great deal can often still be achieved without any drilling.
Sensitivity to sweet and cold
A brief twinge from sweets, cold drinks or ice can indicate that the enamel has been attacked. If the sensitivity persists, it is worth having it looked at.
A visible hole or a rough edge
If your tongue finds a rough patch, or a small hole becomes visible, the decay has usually progressed further. The tooth should be dealt with before long.
Food that always gets stuck
If something keeps catching between the same two teeth, decay or a leaking filling margin can be the reason. That too is easy to check.
Bad breath or a bad taste
Persistent bad breath despite brushing can come from bacteria sitting in a cavity. Combined with other signs, that is a good reason to make an appointment.

Not every discolouration is decay. But white spots, rough patches or dark margins are worth taking seriously, especially if they are new or changing.

The tooth reacts to cold or sweet things

Decay often does not hurt at the start. A brief twinge from something cold or sweet, or when chewing, can still be a hint that the tooth should be examined more closely.

Decay despite brushing, in children, or under old fillings

Sometimes decay appears despite good oral hygiene — between the teeth, in the fissures, or along the margins of old fillings. With children too, an early and unhurried check-up is worthwhile.

Does decay always mean drilling? Not necessarily. How we proceed depends on how deep it goes. This is what a typical course of treatment looks like with us.

    Examination and 3D imaging

    First we look at the tooth closely — using a microscope and digital imaging where needed. That shows us how far the decay reaches and whether the nerve is involved.

    Going through the options together

    We show you what we have found and explain in plain language which steps make sense. With very early decay, gentle remineralisation without drilling can also be an option.

    Gentle anaesthetic if you want it

    To keep the treatment as comfortable as possible, we numb the area locally if you wish. For anxious patients, nitrous oxide or sedation is also available.

    Removing the decay, sparing the tooth

    We remove the diseased tissue selectively and keep as much healthy tooth substance as we can. Clean margins matter, so that the filling seals properly afterwards.

    Restoring the tooth with a filling

    We close the defect with a suitable, usually tooth-coloured filling. Since amalgam has not been permitted in the EU since 1 January 2025, tooth-coloured materials are now the standard everywhere, including the back teeth. For larger defects we can advise you on inlays as an alternative.

    Finally we give you straightforward advice for looking after your teeth at home and discuss whether a professional cleaning would be useful — so that new decay does not get a chance to start.

Costs in Wuppertal

In the great majority of cases, no. Decay caught early can as a rule be dealt with well by a filling. Even where it has reached the nerve, we can often keep the tooth with a root canal treatment. Keeping your own teeth is always what we aim for.

The first consultation is free (€0). A medically necessary filling is as a rule covered by the statutory health insurance (gesetzliche Krankenversicherung, GKV) within standard care (Regelversorgung). This is worth knowing if you have moved to Germany recently: amalgam has been banned across the EU since 1 January 2025, so for back teeth the standard the fund pays for is now a self-adhesive tooth-coloured material, with a bulk-fill composite in exceptional cases. Your entitlement to a filling at no extra charge is unaffected. If you choose a higher-grade material or restoration, there can be an amount for you to pay — that is only ever agreed in writing beforehand, and we set the figure out in a cost plan before we start.

For most adults two check-ups a year is a good guide, and more often if your risk of decay is raised. At these appointments we often catch decay that is just starting, before it causes any trouble. That normally keeps the treatment small and gentle.
