Saving a tooth instead of pulling it — is that possible?

When a tooth is inflamed, or has already had a root canal once, some practitioners move quickly to extraction. Often there is an alternative: under the microscope even fine, hard-to-reach root canals can be seen and cleaned thoroughly. That means teeth can often be kept that at first look beyond saving. There is no guarantee — but in many cases the tooth can be saved. Whether that applies to you is something we work out individually, honestly and without pressure.

As a rule your own tooth is the best solution — it stays firmly rooted and feels natural. So we look carefully at whether your tooth can be kept before any conversation about taking it out.

Why a second opinion is worth having

Keep the tooth, or an implant?

An implant is a good replacement, but the natural tooth usually remains first choice. We weigh it up with you honestly: when an attempt to save the tooth is worth making, and when replacing it is the more sensible route.

It gets harder with deep vertical cracks, severe bone loss, or a crown that is largely destroyed. What is realistic in your case we judge after a proper examination — with 3D imaging where that is indicated — and we will tell you honestly if keeping the tooth does not make sense.

Examination and 3D imaging
First we assess the tooth closely — with a 3D scan (DVT/CBCT) where that is indicated. It shows us how the root canals run and lets us judge whether saving the tooth is worth attempting. A 3D scan is only taken on a justified medical indication, under the applicable radiation protection rules.
An honest assessment and full explanation
We show you the findings and talk openly about what the chances are and what the alternatives would be. You decide how to proceed in your own time and without pressure.
Gentle anaesthesia
The tooth is anaesthetised locally, so that as a rule you should feel little or nothing during treatment. For anxious patients nitrous oxide or sedation can be added on request.
Treatment under the microscope
Under high magnification we clean the fine canals, remove inflamed tissue and find canals that are hard to reach. That precision improves the chance of keeping the tooth. Work is carried out under rubber dam, which keeps the canal system dry and free of saliva.
A tight seal
Once the canals are clean we seal them tightly, so that no bacteria are left behind. That step matters a great deal for keeping the tooth in the long run.
Protection and aftercare
A root-treated tooth is often stabilised with a crown afterwards — we will advise you on that individually. We then go through the aftercare, so that your tooth stays with you as long as possible.

Does my tooth really have to come out?

Not necessarily. Even if you have been advised to have it pulled, a tooth can often still be kept under the microscope — provided enough healthy substance is left. There is no guarantee, but a second opinion brings clarity. In the free first consultation we look at your case without rushing.

What can the microscope do that standard treatment cannot?

The microscope magnifies the inside of the tooth strongly and makes fine structures visible that are hard to see with the naked eye. Hidden or curved root canals can be found and cleaned thoroughly. In many cases that improves the chance of keeping the tooth.

What are the chances of keeping the tooth?

That depends heavily on the individual case — on how much healthy tooth substance is left, and how firmly the tooth sits in the jaw. As a rule the outlook can only be assessed realistically after a proper examination. We do not make promises; we give you an honest assessment of your situation.

Does a root canal under the microscope hurt?

As a rule you should feel little or nothing. The tooth is anaesthetised locally, so most patients notice pressure at most. For anxious patients we can add nitrous oxide or sedation on request, so that the appointment is as relaxed as possible.

Keep my own tooth, or go for an implant?

An implant is a high-quality replacement, but the natural tooth usually stays first choice as long as it can be kept. Which route makes more sense for you is something we weigh up together, honestly. If keeping the tooth is not possible, we are glad to advise on implants as well.
