Toothache is not all the same. How it feels often gives a first clue to the cause. This overview does not replace an examination, but it can help you place the pain — the precise diagnosis is one we reach together at the practice.
A twinge with sweet, cold or hot things
A short, sharp pain with something sweet, cold or hot often points to decay or exposed tooth necks. Here we check whether the decay needs treating, or whether the sensitivity itself can be eased in a targeted way.
Throbbing pain that does not let up
Knocking, pulsing pain that carries on through the night can point to an inflamed tooth nerve. Often the tooth can still be saved with root canal treatment — the sooner, the better.
Pain when chewing or biting down
If one particular tooth only hurts when you bite, deep decay, a crack or an old leaking filling may be behind it. We establish the cause and restore the tooth accordingly, for example with a new filling.
Pain at the back of the jaw
Pressure and pain right at the back of the jaw, often with swollen gum over it, are frequently linked to a wisdom tooth coming through. We look at its position and discuss calmly whether and when anything needs doing.
Bleeding, swollen gums
When it is not a single tooth but the gum that hurts, bleeds and looks red, gum inflammation is often the reason. Treated early, it can as a rule be brought under control well.

Severe throbbing pain, swelling, fever, pus, an accident or a swollen cheek should be looked at by a dentist the same day. Swelling that spreads towards the eye or the floor of the mouth, or that makes swallowing or breathing difficult, belongs in a hospital emergency department, not in a waiting room. In our region it is run by the Nordrhein dental chamber and covers the hours when practices are closed; ours are Mon–Fri 7:30–19:30 and Sat 9:00–15:00. You reach the service on 01805 98 67 00, through the search on the chamber’s website, or via the DEMedic app. One thing worth knowing: 116 117 is the medical on-call line, not the dental one. The on-call practice is not staffed continuously, so always ring before you set off, and take your health insurance card (eGK) with you.
